Re: Complimentary oral health screenings for residents in long-term care through the Home Dental Care Program

Dear families:

The University of Manitoba’s Centre for Community Oral Health (CCOH) is pleased to offer eligible individuals living in long-term care a complimentary oral health assessment through the Home Dental Care Program. Thanks to a generous donation from Green Shields Canada, CCOH will be able to expand access to oral health care for residents living in long-term care facilities. Oral health assessments will be performed by CCOH dentists and dental hygienists. The complimentary assessment may be available for new patients or existing patients who have not received an examination or dental treatment in the last 18 months. The oral health assessment will be used to identify the patient’s oral health status and oral health care needs.

Please see the link to the enrolment form ( https://umanitoba.ca/dentistry/sites/dentistry/files/2023-05/HomeDentalCare-EnrolmentForm.pdf )  that residents and/or their Power of Attorney (POA) can fill out to enroll in the program. Once the form is submitted to the Home Dental Care office the individual will be placed on a list for the next available date when the dental or dental hygiene team will go to the facility and provide the oral health assessment. After the oral health assessment has been completed the oral health status along with any treatment needs and recommendations will be provided to the resident, POA and nursing staff.

Please contact the Home Dental Care team with any questions.

We look forward to working with you, together to help improve access to dental care service and improving the oral health of residents living in long term care.

Sincerely,

The Home Dental Care Program Team
